HOW DO I SUBMIT THE TEST RESULT/S ?

To have a test result included in the Health Test Results Register we require 3 documents: 1. A photocopy of the test certificate, laboratory report or, for Patellar Luxation, the written opinion of a veterinarian or appointed breeder-assessor, and 2. An original Statutary Declaration, witnessed by a Justice of the Peace, that - it is a true and correct record of the certificate/report, and - the person submitting the certificate/report is or was the legal owner of the dog at the time of the testing (the Statutary Declaration form is available from any newsagency), and 3. A photocopy of the certified pedigree of the tested dog.
